Here's what really happened. During last turn, First roll was 6 and 3. Second roll was 6 and 12. Third roll was 6. Forth roll was 6 and 6. The forth roll is what I saw on my screen. On SpaceDealer's side he got the error at this point and pressed f5. Then he got the roll or stop so he rolled again and got 12 instead of 6 and 6. Then of course he rolled again and won.
I've seen this happen before in my can't stop games over the last couple of weeks but the effect has been minor. This is the first time where the bug led to an unfair loss (he would not have rolled again after taking the six column with the original 4th roll)

Unexpected error: Error with notification service (TIMEOUT), please retry
followed by another message to press f5. If you don't press f5 but try to choose one of the rolled numbers you get a message "This game action is impossible right now"
I press f5 and the game re-rolls.
Also, I'm currently inactive? How can that be? I had a white dot in my last game. What is going on?
As you may have noticed, we manage to fix the issue temporarily on July 28th.
The problem got back on August 5th (with a maximum on August 12th).
Then we manage to set up an "almost permanent" fix. Since August 13th, there is very few errors of this type. As there hasn't been additional comments on this thread since August 12th it looks like we found the cause.
We are working on a last fix to clean the latest remaining "timeout" errors.
